Sut rydyn ni'n dal ac yn prosesu'ch gwybodaeth

Penodwyd Wavehill gan HCC i gynnal arolwg o agweddau milfeddygon tuag at gynllunio iechyd rhagweithiol gyda ffermwyr cig eidion a defaid.

Hoffem siarad â milfeddygon ledled Cymru i gasglu eu barn a'u safbwyntiau ar gynllunio iechyd rhagweithiol. Byddwn yn defnyddio'r wybodaeth a gasglwyd yn yr holiadur i ysgrifennu adroddiad ar gyfer HCC, a fydd yn cael ei ddefnyddio i lywio sut y gallai diwydiant a'r proffesiwn milfeddygol gefnogi cynllunio iechyd rhagweithiol yng Nghymru.

Casglwyd eich manylion cyswllt o wybodaeth sydd ar gael i'r cyhoedd, gan gynnwys gwefannau Milfeddygon. Mae cymryd rhan yn yr holiadur yn hollol wirfoddol; gallwch benderfynu peidio â chymryd rhan, neu gallwch ddewis peidio ag ateb rhai cwestiynau os yw'n well gennych.

Nod yr ymchwil yw ymchwilio barnau a safbwyntiau milfeddygon tuag at gynlluniau iechyd rhagweithiol. Y bwriad yw cefnogi gweithredoedd a allai wella ac ehangu cynllunio iechyd rhagweithiol ar gyfer diwydiant ac ar gyfer milfeddygon.
 

2. Pa fath o wybodaeth sy'n cael ei chasglu trwy'r arolwg?

Mae'r arolwg yn canolbwyntio ar eich barn a'ch safbwyntiau tuag at ddarparu cynlluniau iechyd rhagweithiol gyda ffermwyr cig eidion a defaid. 3. Beth yw data personol?

Mae data personol yn golygu unrhyw wybodaeth a allai arwain at adnabod unigolyn naill ai ar ei ben ei hun neu mewn cyfuniad â gwybodaeth arall sydd ar gael e.e. enw, cyfeiriad, neu fanylion sy'n benodol i'r person hwnnw.

4. Am ba hyd y cedwir data personol?

Bydd Wavehill yn cadw data personol yn ystod cyfnod y contract ond byddent yn dileu unrhyw wybodaeth bersonol o fewn 3 mis i ddiwedd yr arolwg.

5. Beth yw pwrpas prosesu'ch atebion i'r holiadur?

Defnyddir y data at ddibenion ymchwil yn unig. Dadansoddir y data i alluogi'r HCC i ddeall barn a safbwyntiau'r gymuned filfeddygon tuag at gynllunio iechyd rhagweithiol. Bydd hyn yn helpu i benderfynu sut y gallai diwydiant annog a chefnogi mwy o ymgysylltiad â chynllunio iechyd. Ni fydd y data'n cael ei ddefnyddio at ddibenion masnachol neu farchnata ac ni chaiff ei ddefnyddio i wneud penderfyniadau amdanoch chi fel unigolyn neu arfer eich safle gwaith.

6. Pwy sydd â mynediad at y data personol a gasglwyd trwy'r arolwg?

Bydd gan Wavehill gopi o'r data personol i'w ganiatáu i gynnal yr arolwg a bydd yn dal enw, enw'r sefydliad, rhif ffôn ac e-bost yn ystod yr arolwg hwn.

Ni fydd yr adroddiad hwn yn nodi unrhyw unigolion sy'n cymryd rhan yn yr ymchwil. Bydd Wavehill yn dileu'r holl ddata personol o fewn tri mis i ddiwedd yr arolwg. Ni fydd gan HCC fynediad at ddata personol a gesglir trwy'r holiaduron.

How we hold and process your information

Wavehill have been appointed by HCC to carry out a survey of veterinarian’s attitudes towards proactive health planning with beef and sheep farmers.

 

We would like to speak to vets across Wales to gather their views and perspectives on proactive health planning. We will use the information captured in survey to write a report for HCC, which will be used to inform how industry and the veterinary profession could support proactive health planning in Wales.

 

Your contact details have been gathered from public facing information, including from vet Practice websites. Participation is voluntary; you can decide not to take part, or you can choose not to answer certain questions if you prefer.

 

The information you provide will only be used for the purposes of this research. Comments that you make will be confidential and will not be attributed to you unless we have your express permission to do so. All data will however be anonymised and Wavehill will delete your contact information from our systems within three months of the end of the survey.

1. Why is this research happening?

The aim of the research is to explore the views and perspectives of vets towards proactive health planning. It is intended to support actions that could improve and expand proactive health planning for industry and for vets.
 

2. What type of information is collected through the survey?

The survey focuses on your views and perspectives towards providing proactive health planning with beef and sheep farmers. The survey also captures other relevant information, including your involvement with proactive health planning, and the factors that either support or constrain you in engaging with health planning.

 

3. What is personal data?

Personal data means any information that could lead to a person being identified either alone or in combination with other widely-available information, e.g. their name, their address, or details specific to that person.

4. How long will personal data be kept for?

Wavehill will hold personal data during the contract period but will delete it within three months of the end of the survey.

5. What is the purpose of processing your answers to the survey?

The data is used for research purposes only. The data will be analysed to enable HCC to understand the views and perspectives of the vet community towards proactive health planning. This will help to decide how industry could encourage and support greater engagement with health planning. The data will not be used for commercial or marketing purposes and it will not be used to take make decisions about you as an individual or your vet practice.

6. Who has access to the personal data collected through the survey?

Wavehill will have a copy of the personal data to allow it to carry out the survey and will capture name, organisation name, telephone number and email during this survey.

This report will not identify any individuals participating in the research. Wavehill will delete all personal data within three months of the end of the survey. HCC will not have access to personal data collected through the interviews.
