Holiadur prosiectau LEADER Ceredigion

Sut rydym yn storio ac yn prosesu eich gwybodaeth

Mae Cyngor Sir Ceredigion, ar ran Cynnal y Cardi Local Action Group, wedi comisiynu Wavehill i gynnal gwerthusiad annibynnol o weithrediad a deilliannau’r rhaglen LEADER yn Sir Ceredigion.

Fel rhan o’r gwerthusiad, rydym yn edrych i siarad gyda sefydliadau wnaeth gais am gyllid gan y rhaglen. Rhoddwyd eich manylion cyswllt i ni gan Gyngor Sir Ceredigion ar y sail eich bod wedi cytuno i gymryd rhan yn y gwerthusiad. Mae cymryd rhan yn gwbl wirfoddol, fodd bynnag.

Yn y bôn, rydym yn edrych ar ba mor dda y rheolwyd y rhaglen a pha wahaniaeth mae hi’n wneud yn Sir Ceredigion. Rydym yn gofyn am adborth ar y broses gais ac, os ariannwyd eich prosiect, ychydig o gwestiynau ynglŷn â beth mae eich prosiect wedi ei gyflawni hyd yma.

Cynhelir y cyfweliad hwn fel rhan o’r broses werthuso canol-tymor, a fydd yn adrodd nôl diwedd mis Rhagfyr 2018. Bydd yr adroddiad terfynol yn cael ei gynhyrchu ym mis Ebrill 2021.

Bydd y wybodaeth a ddarparwch yn cael ei defnyddio at ddibenion ymchwil a gwerthuso yn unig. Cedwir unrhyw wybodaeth bersonol a gesglir fel rhan o’r gwerthusiad yn gwbl gyfrinachol. Ni chaiff eich atebion i’r arolwg eu gwneud yn gyhoeddus mewn unrhyw ffordd a fyddai’n ei gwneud hi’n bosib i’ch adnabod chi neu eich sefydliad. Bydd Wavehill yn cynhyrchu adroddiad ar sail y data ond ni fydd yr adroddiad hwn yn enwi unrhyw unigolion na sefydliadau.

Bydd Wavehill yn dileu’r wybodaeth a ddarparwch a’r holl ddata personol sy’n ymwneud â’r ymchwil hon o fewn tri mis i derfyn y gwerthusiad.

Os oes gennych chi unrhyw gwestiynau ynglŷn â’r cyfweliad neu’r gwerthusiad yn fwy cyffredinol, cysylltwch ag Endaf Griffiths yn Wavehill (endaf.griffiths@wavehill.com | 01545 571711) neu Meleri Richards (Meleri.Richards@ceredigion.gov.uk).

Gallwch hefyd gysylltu â swyddog diogelu data Llywodraeth Cymru ar DataProtectionOfficer@gov.wales.

O dan y ddeddfwriaeth diogelu data newydd, mae gennych yr hawl:

  • I gael mynediad at eich data personol a gedwir gan Gyngor Sir Ceredigion.

  • I fynnu bod Cyngor Sir Ceredigion yn cywiro unrhyw gamgymeriadau sydd yn y data.

  • I wrthwynebu (mewn rhai amgylchiadau) neu gyfyngu ar brosesu.

  • I’ch data gael ei ‘dileu’ (mewn rhai amgylchiadau).

Cysylltwch â Meleri Richards os hoffech wneud unrhyw un o’r rhain mewn perthynas â’r prosiect hwn.

Os oes gennych unrhyw bryderon ynghylch sut mae eich data wedi cael ei drin, gallwch gyflwyno cwyn i Swyddfa’r Comisiynydd Gwybodaeth sy’n rheoleiddiwr annibynnol Llywodraeth Cymru ar gyfer diogelu data. Gallwch gysylltu â Swyddfa’r Comisiynydd Gwybodaeth ar 01625 545 745 neu 0303 123 1113, drwy’r wefan www.ico.gov.uk neu ysgrifennwch at: Information Commissioner, Wycliffe House, Water Lane, Wilmslow, Cheshire, SK9 5AF.

Gwybodaeth bellach

1. Pam mae’r gwaith ymchwil hwn yn digwydd?

Nod y gwerthusiad yw asesu pa mor dda y cafodd y rhaglen LEADER ei rheoli a pha wahaniaeth mae hi’n ei wneud yn Sir Ceredigion.

Rydym yn edrych i siarad gyda sefydliadau wnaeth gais am gyllid gan y rhaglen er mwyn cael adborth ar y broses gais ac, os derbyniwyd cyllid, er mwyn canfod beth mae’r prosiect wedi ei gyflawni.

2. Pa fath o wybodaeth a gesglir drwy gyfrwng yr arolwg?

Mae’r cyfweliad yn ffocysu ar eich profiad chi o’r broses gais a’r gefnogaeth/cyllid sydd ar gael drwy’r rhaglen LEADER.

3. Beth yw data personol?

Mae data personol yn golygu unrhyw wybodaeth a allai arwain at adnabod person, naill ai ar ei phen ei hun neu ar y cyd â gwybodaeth arall sydd ar gael yn eang, e.e. enw person, ei gyfeiriad, neu fanylion sy’n benodol am y person hwnnw.

Mae Wavehill yn defnyddio data personol a ddarparwyd gan Gyngor Sir Ceredigion i hwyluso’r arolwg hwn, e.e. eich enw a’ch manylion cyswllt.

4. Am ba hyd y cedwir data personol?

Bydd Wavehill yn cadw data personol yn ystod cyfnod y cytundeb, ond bydd yn ei ddileu dri mis ar ôl i’r cytundeb ddod i ben.

5. Beth yw’r sail gyfreithiol ar gyfer casglu a phrosesu data a gasglwyd yn yr arolwg?

Mae gwerthusiad y rhaglen LEADER yn galluogi Cyngor Sir Ceredigion a’r Cynnal y Cardi Local Action Group i ddeall a yw’r rhaglen yn gweithio’n effeithiol. Felly fe’i defnyddir i gefnogi gweithgareddau o fewn tasg gyhoeddus Cyngor Sir Ceredigion. Hynny yw, rôl graidd a swyddogaethau’r Cyngor. Er enghraifft, gall y wybodaeth a gesglir gael ei defnyddio er mwyn:

  • Penderfynu a oes angen gwneud newidiadau i’r rhaglen LEADER

  • Penderfynu a ddylai cynlluniau fel y rhaglen LEADER barhau yn y dyfodol

  • Deall y ffyrdd gorau o gefnogi sefydliadau yn Sir Ceredigion

Comisiynwyd y gwerthusiad gan Gyngor Sir Ceredigion, ar ran Cynnal y Cardi Local Action Group. Mae eich cyfranogiad unigol yn yr ymchwil yn wirfoddol.

6. Beth yw pwrpas prosesu eich atebion i’r arolwg?

Defnyddir y data at ddibenion ymchwil a gwerthuso yn unig. Bydd y data yn cael ei ddadansoddi er mwyn galluogi Cyngor Sir Ceredigion a Cynnal y Cardi Local Action Group i ddod i ddeall effaith ac effeithiolrwydd y rhaglen LEADER. Bydd hyn yn helpu wrth benderfynu a oes angen gwneud unrhyw newidiadau i’r rhaglen LEADER, a sut y dylid datblygu’r gefnogaeth a gynigir i sefydliadau yn y dyfodol. Ni fydd y data yn cael ei ddefnyddio at ddibenion masnachol na dibenion marchnata, ac ni fydd yn cael ei ddefnyddio i gymryd penderfyniadau amdanoch chi fel unigolyn na’ch sefydliad.

7.Pwy sydd â mynediad at y data personol a gasglwyd drwy’r arolwg?

Bydd gan Wavehill gopi o’r data personol i’w alluogi i gynnal yr arolwg. Bydd yn dileu data personol o fewn tri mis i’r prosiect ddod i ben.

Bydd Wavehill yn paratoi cofnod ysgrifenedig o’r drafodaeth yn ystod y cyfweliad. Mae’n bosib y bydd cyfweliadau hefyd yn cael eu recordio’n ddigidol i hwyluso’r broses o gymryd nodiadau (gofynnir eich caniatâd os digwydd hynny). Bydd y nodiadau/recordiadau hyn yn cael eu dadansoddi gan ymchwilwyr Wavehill i lunio adroddiad i Gyngor Sir Ceredigion a’r Cynnal y Cardi Local Action Group. Ni fydd yr adroddiad hwn yn enwi unrhyw unigolion sy’n cymryd rhan yn yr ymchwil ac ni fydd y nodiadau/recordiadau yn cael eu rhannu â Chyngor Sir Ceredigion, Cynnal y Cardi Local Action Group, na neb arall tu allan i Wavehill. Bydd Wavehill yn dileu’r holl ddata personol o fewn tri mis i’r prosiect ddod i ben. Ni fydd Cyngor Sir Ceredigion na Cynnal y Cardi Local Action Group yn cael mynediad at y data personol a gasglwyd drwy’r cyfweliadau.

How we hold and process your information

Wavehill have been commissioned by Ceredigion County Council, on behalf of Cynnal y Cardi Local Action Group, to undertake an independent evaluation of the implementation and outcomes of the LEADER programme in Ceredigion.

As part of the evaluation we are looking to speak to organisations that applied for funding from the programme. Your contact details were provided to us by Ceredigion County Council on the basis that you have agreed to participate in the evaluation. Participation is however voluntary.

Essentially, we’re looking at how well the programme has been managed and what difference it is making in Ceredigion. We’re asking for feedback on the application process and, if your project was funded, a few questions about what your project has achieved to date.

This interview is being undertaken as part of the mid-term evaluation process, due to report by the end of December 2018. The final report is scheduled to be produced in April 2021.

The data you provide will only be used for research and evaluation purposes. Any personal information collected as part of the evaluation is kept confidential. Your answers to the survey will not be made public in a way that could lead to you or your organisation being identified. Wavehill will produce a report based on the data but this will not identify any individuals or organisations.

Wavehill will delete the information you provide and all personal data relating to this research within three months of the end of the evaluation.

If you have any questions regarding the interview or the evaluation more generally, please contact Endaf Griffiths at Wavehill (endaf.griffiths@wavehill.com | 01545 571711) or Meleri Richards   (Meleri.Richards@ceredigion.gov.uk).

You can also contact the data protection officer for the Welsh Government at DataProtectionOfficer@gov.wales.

  • To (in certain circumstances) object to or restrict processing.

  • For (in certain circumstances) your data to be ‘erased’.

Please contact Meleri Richards if you wish to do any of these things in relation to this project.

If you have any concerns about how your data has been handled, you can lodge a complaint with the Information Commissioner’s Office who is Welsh Government’s independent regulator for data protection.
