Mae Wavehill yn gwmni ymchwil annibynnol ac rydym wedi cael ein comisiynu gan Gyngor Bwrdeistref Sirol Rhondda Cynon Taf i gynnal gwerthusiad o'r prosiect Cadw’n Iach yn y Gwaith. Prif nod y gwerthusiad yw i ddeall sut mae'r prosiect wedi delifero ar amcanion rhanddeiliaid allweddol. Bydd hyn yn cynnwys deall a) effaith prosiectau ar fusnesau yn ogystal â chyfranogwyr unigol, b) cost effeithiolrwydd a gwerth am arian o gymharu â phrosiectau eraill sy'n ceisio canlyniadau tebyg, ac c) gwersi a ddysgwyd, arfer gorau ac argymhellion ar gyfer prosiectau yn y dyfodol.

Bydd Wavehill yn cynnal dau gam gwerthuso hyd at fis Ebrill 2023. Bydd adroddiad gwerthuso interim yn cael ei gynhyrchu ym mis Ebrill 2022 yn canolbwyntio ar effeithiau cynnar a chynnydd yn erbyn nodau yn ogystal â pherfformiad cyflawni ac argymhellion ar gyfer cyflawni yn y dyfodol. Bydd yr adroddiad gwerthuso terfynol yn cael ei gynhyrchu ym mis Ebrill 2023 a bydd yn darparu asesiad crynodol o effaith ynghyd ag argymhellion ar gyfer unrhyw gyflawni yn y dyfodol.

Fel rhan o’r gwerthusiad bydd Wavehill yn ceisio cynnal arolwg / cyfweliadau ffôn gyda busnesau ac unigolion sydd wedi cymryd rhan yn y prosiect (h.y. y buddiolwyr) er mwyn deall sut maent wedi elwa (os o gwbl) a boddhad â’r cymorth a dderbyniwyd.

Hoffem siarad â buddiolwyr y rhaglen a rhanddeiliaid eraill i gael eich barn ar reolaeth a darpariaeth y rhaglen, yn ogystal ag unrhyw ganlyniadau a allai fod wedi codi o'r gefnogaeth a dderbyniwyd neu unrhyw effaith mae'r gefnogaeth wedi cael ar y sector. Mae’r tîm Cadw’n Iach yn y Gwaith wedi rhoi manylion y busnesau sydd wedi cymryd rhan yn y prosiect i ni, tra nad oes unrhyw wybodaeth am gyfranogwyr unigol wedi’i rhannu.

Mae'r cyfweliad yn canolbwyntio ar gipio gwybodaeth gefndirol am eich busnes, y rhesymau yr oeddech eisiau dderbyn cefnogaeth o Cadw’n Iach yn y Gwaith, eich profiad o'r gefnogaeth ac unrhyw ganlyniadau.

  3.   Beth yw data personnol?

Mae data personol yn golygu unrhyw wybodaeth a allai arwain at adnabod person, nail ai ar ei phen ei hun neu ar y cyd a gwybodaeth arall sydd ar gael yn eang, e.e. enw person, ei gyfeiriad, neu fanylion sy’n benodol am y person hwnnw.

4.  Am ba hyd y cedwir data personol?

Bydd Wavehill yn cadw data personol yn ystod cyfnod y contract a bydd y data yn cael ei ddileu chwe mis ar ôl diwedd y contract ym mis Ebrill 2023.

5. Pa mor ddiogel yw eich data personol?

Mae gwybodaeth bersonol a ddarperir i Wavehill bob amser yn cael ei storio ar weinydd diogel. Dim ond nifer cyfyngedig o ymchwilwyr sy'n gweithio ar y prosiect hwn sy'n gallu cyrchu'r data. 6.  Beth yw'r sylfaen gyfreithiol ar gyfer casglu a phrosesu'r data a gasglwyd trwy'r arolwg?

Mae'r gwerthusiad yn cael ei gynnal gan Wavehill ar ran Cyngor Bwrdeistref Sirol Rhondda Cynon Taf. Mae eich cyfranogiad unigol yn yr ymchwil yn wirfoddol ac felly mae'n seiliedig ar eich caniatâd

7.   Beth yw pwrpas prosesu'ch atebion i'r arolwg?

Defnyddir y data at ddibenion ymchwil a gwerthuso yn unig. Dadansoddir y data i alluogi Cyngor Bwrdeistref Sirol Rhondda Cynon Taf i ddeall effaith ac effeithiolrwydd prosiect Cadw'n Iach yn y Gwaith. Bydd hyn yn helpu'r Cyngor Bwrdeistref Sirol Rhondda Cynon Taf i benderfynu a oes angen gwneud unrhyw newidiadau ac i sefydlu a ddylai'r rhaglen hon barhau yn y dyfodol. Bydd gan Wavehill gopi o'r data personol i'w alluogi i gynnal yr arolwg. Bydd Wavehill yn gwneud cofnod ysgrifenedig o'r drafodaeth yn ystod y cyfweliad. Dadansoddir y nodiadau hyn gan ymchwilwyr yn Wavehill i gynhyrchu adroddiad ar gyfer Cyngor Bwrdeistref Sirol Rhondda Cynon Ta. Ni fydd yr adroddiad hwn yn nodi unrhyw unigolion sy'n cymryd rhan yn yr ymchwil heb gydsyniad ymlaen llaw ac ni fydd nodiadau'r cyfweliadau yn cael eu rhannu â Cyngor Bwrdeistref Sirol Rhondda Cynon Tanac unrhyw un arall y tu allan i Wavehill. Bydd Wavehill yn dileu'r holl ddata personol mewn chwe mis ar ôl diwedd y prosiect.

Staying Well at Work Privacy Notice

Wavehill is an independent research company and we have been commissioned by Rhondda Cynon Taf County Borough Council to undertake an evaluation of the Staying Well at Work project. The main goal of the evaluation is to identify how it meets the objectives of key stakeholders. This will include understanding a) project impact on SMEs as well as individual participants, b) cost effectiveness and value for money in comparison with other projects seeking similar outputs, and c) lessons learnt, best practice and recommendations for future projects.

Wavehill will be conducting two evaluation phases up to April 2023. An Interim evaluation report will be produced in April 2022 focussing on early impacts and progress against aims as well as delivery performance and recommendations for future delivery. The final evaluation report will be produced in April 2023 and will provide a summative assessment of impact along with recommendations for any future delivery.

As part of the evaluation Wavehill will seek to conduct a survey / telephone interviews with businesses and individuals who have participated in the project (i.e. the beneficiaries) in order to understand how they have benefitted (if at all) and satisfaction with the support received.

How we hold and process your information 

We would like to speak to beneficiaries of the programme and other stakeholders to capture views about how it was managed and delivered, as well as any outcomes that may have arisen from the support received / outcomes for the sector. The Staying Well at Work team have provided us with details of the businesses who have participated in the project, although no information about individual participants have been shared. . 

The data you provide will only be used for the purposes of this evaluation.  Any personal information collected as part of the evaluation is kept confidential.  Your answers to the interview will not be made public in a way that could lead to you being identified.  Wavehill will produce a report based on the information but this will not identify any individuals without prior consent. 

It is also important to note that the team undertaking the evaluation do not work for Rhondda Cynon Taf County Borough Council or any of the organisations that are involved in the delivery or funding of the Staying Well at Work project.  This is an independent evaluation. 

Wavehill will delete the information you provide and all personal data relating to this research within six months of the end of the evaluation.

Why is this research happening?

It is a requirement for all projects funded through the Wales European Funding Office to undergo an external, independent evaluation in order to identify impacts and generate learning that can inform future delivery. This evaluation is being undertaken to understand what has been achieved as well as to support the ongoing delivery of the project by providing findings ‘in-flight’ that can be used to inform the decisions made by the team.

 2.  What type of information is collected through the survey? 
 

The interviews and survey with businesses and individuals focuses on capturing background information (including health issues), reasons you wanted to receive support from  Staying Well at Work, your experience of the support received and any outcomes received. 

3.  What is personal data? 
  

Personal data means any information that could lead to a person being identified either alone or in combination with other widely available information, e.g. their name, their address, or details specific to that person. 

4.  How long will personal data be kept for? 
  

Wavehill will hold personal data during the contract period but will delete it six months after the end of the contract in April 2023. 

6.   How secure is your personal data?

Personal information provided to Wavehill is always stored on a secure server. The data can only be accessed by a limited number of researchers working on this project. Wavehill will only use this data for research purposes. Wavehill has cyber essentials certification.

Wavehill has procedures to deal with any suspected data security breaches. If a suspected breach occurs, Wavehill will report this to Rhondda Cynon Taf County Borough Council who will notify you and any applicable regulator where we are legally required to do so.

6. What is the legal basis for collecting and processing the data collected through the survey? 

 
The evaluation is being carried out by Wavehill on behalf of Rhondda Cynon Taf County Borough Council.  Your individual participation in the research is voluntary and is therefore based on your consent. 

7.  What is the purpose of processing your answers to the survey? 
  

The data is used for research and evaluation purposes only.  The data will be analysed to enable Rhondda Cynon Taf County Borough Council to understand the impact and effectiveness of the  project.  This will help Rhondda Cynon Taf County Borough Council decide whether any changes need to be made and to establish if this programme should continue in the future.  The data will not be used for commercial or marketing purposes and it will not be used to take make decisions about you as an individual. 
 
8.  Who has access to the personal data collected through the survey? 

Wavehill will have a copy of the personal data collected through the survey.  Wavehill will make a written record of the discussion during the interview.  These notes will be analysed by researchers at Wavehill to produce a report for Rhondda Cynon Taf County Borough Council. This report will not identify any individuals participating in the research without prior consent and the notes of the interviews will not be shared with Rhondda Cynon Taf County Borough Council or anyone else outside of Wavehill.  Wavehill will delete all personal data within six months of the end of the project.
